Information About Family Members


  • Social Security Numbers: Bring Social Security cards or written proof from the Social Security Service Administration, failure to do so will result in termination of rental assistance.
  • Identification: State issued I.D. for all household member(s) 18 and over, birth certificates, custody agreements, adoption paper, etc..
  • Full Time Student Status: If any family members are 18 years of age or older and attending school full-time bring the name and address of the school they are attending.
  • Handicap or Disabled: If any member of your family is handicapped or disabled bring in information about any income the family member receives due to the handicap or disability.

Information About Your Income and Assets


  • Employment Income: For every member of your family that works please bring in the following information.
  1. Name, Address and telephone number of the employer.
  2. Current rate of regular pay, overtime pay and the number of hours normally worked.
  3. Information about any changes you expect to receive from employment such as tips, commissions, profit sharing, etc...
  • Benefit and Support Income: If any member of your family receives any of the following types of income bring the name, address and telephone number of the source of income and information about the amount received.
  1. Unemployment Compensation
  2. Social Security
  3. Supplemental Social Security
  4. Pension
  5. Disability Income
  6. Alimony
  7. Child Support
  8. Welfare or other Public Assistance
  9. Regular Support from Family Members or Friends
  • Checking and Savings Accounts including Christmas Clubs, Certificates of Deposits, IRA's and Keogh Account. Please bring the name and address of your bank, account numbers and balances for all of the accounts.
  • Real Estate: Bring information about the current value of any property owned by a family member. If you have rental property bring information regarding income and expenses from the rental property.
  • Stocks, Bonds, Trusts and other Investments: Bring account numbers and statements on the value and information about the income from the investments.
  • Life Insurance Policies: Bring the name, policy numbers and address of the life insurance company.
  • Other Income: For any other type of income family members have bring the name, address, amount and telephone numbers of the source of the income.
  • If you have sold or given away any asset in the past two years bring in the information about those assets.

Expenses

  • Bring information about any of the following expenses you expect to have in the next 12 months
  1. Medical expenses not covered by insurance. (Elderly, Handicapped or Disabled Families Only)
  2. Medical insurance premiums or amounts deducted from your pay for medical insurance. (Elderly, Handicapped or Disabled Families Only)
  3. child care expenses to care for your children while you work or go to school
  4. Expenses to care for a handicapped or disabled family member while you work
