Information About Family Members

  • Social Security Numbers: Bring Social Security cards or written proof from the Social Security Administration
  • Identification: State issued I. D. for all household members 18 years of age and older, birth certificates, custody agreements, adoption papers etc...
  • Full Time Student Status: If any family members are 18 years of age or older and attend school full-time bring the name and address of the school they are attending.
  • Handicap or Disabled: If any member of your family is handicapped or disabled bring information about any income the family member receives because of the handicap or disability.

Information About Income And Assets

  • Employment Income: For every member of your family that works bring in the following information:

    • Name, address, and telephone number of the employer
    • Current rate of regular pay, overtime pay and number of hours normally worked
    • Information about any changes you expect to receive from employment as tips, commissions, profit sharing, etc.

  • Benefit and Support Income: If any member of your family receives any of the following types of income bring the name, address and telephone number of the source of the income and information about the amount received:

    • Unemployment Compensation
    • Social Security
    • Supplemental Social Security
    • Pension
    • Disability Income
    • Alimony
    • Child Support
    • Welfare or other Public Assistance
    • Regular support from family members or friends

  • Checking and Savings Accounts: Including Christmas Clubs, Certificates of Deposits (CDs), IRA's and Keogh account. Bring the name and address of your bank, account numbers for all accounts and the balance of your accounts.
  • Real Estate: Bring information about the current value of the property. If you own the property and rent it bring the address of the property, information about the income received and expenses from the property. (Bring last year's Schedule E from your income tax form)
  • Stocks, Bonds, Trusts and other Investments: Bring account numbers and statement(s) validating the income and information about the investments.
  • Other Income: For any other income your family has bring the name, address and telephone number of the source of the income.

If you have sold or given away any asset in the past two years bring in information about those assets.

Expenses

Bring information about any of the following expenses you expect to have in the next twelve months

  • Medical expenses not covered by insurance. (Elderly, Handicapped, or Disabled Families Only)
  • Medical insurance premiums or amounts deducted from your pay for insurance. (Elderly, Handicapped, or Disabled Families Only)
  • Child care expenses
  • Expenses to care for a handicapped or disabled family member while you work
